Apart from significant others and friends, the need for being understood in treatment should be there regardless of how one is able to or not able to communicate; some people seek treatment for this very reason. And sometimes it is easier to be understood by friends and significant others than by therapists, educators, mentors, and other professionals - especially when you are a minority and dealing with systemic trauma, not just interpersonal trauma. I just recall studies on cultural sensitivity in psychology, as well as what Sue and Carter both at Columbia University have researched on microaggressions and Race-Based Traumatic Stress Injury, respectively.
Systemic traumas are ongoing, and it's hard to pinpoint the offenders or etiologies in some cases at least according to some peace studies that involve psychology as well. To be understood, in some cases, means to be able to express to those who might not get it or understand, which makes it challenging for clinicians and clients alike.
Lack of understanding coupled with ongoing systemic trauma does a lot of damage to survivors psyche and neurology, in my humble opinion.

There is a difference, it seems, between interpersonal traumas and systemic traumas, isolated traumas and poly-traumatizations, victimization-based traumas and non-victimization traumas, maltreatment traumas and non-maltreatment traumas, and other forms of traumas. Experiencing trauma and the need to be understood appear to correlate in some meaningful way - but how? It would be interesting to see that talked about, studied, and innovated in research and clinical practice.
The greatest problem I've had in treatment as a veteran and survivor of both military and civilian traumas was the lack of understanding and cultural sensitivity required for treatment on poly-traumatization, or what the VA considers as chronic PTSD. It's just a thought, though I may be biased since I'm a survivor.
